Position title: Director of Population Health, UW Health

Kristen Brey is the Director of Population Health at UW Health.  Kristen holds an Advance Practice Social Work license.  The Office of Population Health has work in three main areas (1) Accountable Care Organization and Value Based Care, (2) Community Health Improvement, and (3) Care Model Development.  Kristen serves as a preceptor for preventative medicine residents who are interested in value-based care and population health management.
